Job Title: Adult Nurse Case Manager Profession: RN Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8:30 AM - 5:00 PM Experience: 2 years RN experience License: Active RN License Certifications: BLS Must-Have: Current in-patient Case Management experience; EPIC and post-acute facility referral platforms experience Description: The Adult Nurse Case Manager coordinates care for a select caseload of patients from triage through safe transfer to home. This role requires proactive communication with payors to ensure approval of treatment plans and necessary resources. The case manager maintains knowledge of clinical systems and fiscal data related to specific patient populations. Collaboration with the multidisciplinary team, patients, and their families is essential for effective transition planning. Responsibilities include coordinating transitions to post-acute settings such as nursing facilities and inpatient rehabilitation. The case manager manages patients according to departmental procedures while maintaining clinical safety and quality. Patient information must be entered and maintained in all departmental systems per operational guidelines. A current/active CPR card is required for this position.
